Diabetes, ulcers and hyperbaric medicine.

Diabetes in many cases leads to ulcers. Hyperbaric oxygen therapy can help.

 

Do sladkorne bolezni, pogosto imenovane tudi DIABETES, pride bodisi zato, ker trebušna slinavka ne proizvaja dovolj inzulina, bodisi zato, ker se telesne celice na proizvedeni inzulin ne odzivajo ustrezno. Če se ne zdravi oziroma je zdravljenje neuspešno, lahko sladkorna bolezen povzroči številne zaplete. Zelo pogosto je to razjeda stopal. Razjeda (ULKUS) je okvara kože in podkožnega tkiva. Pri sladkornih bolnikih se celi zelo počasi, včasih pa se celo noče zaceliti in lahko pripelje tudi do amputacije.

 

Do razjede privedejo poleg omenjenih zapletov sladkorne bolezni (okvara živcev, motnje prekrvitve, spremembe oblike stopala in omejena gibljivost sklepov) tudi naslednji zunanji vzroki:

 

  •  mehanske poškodbe (ureznine, žulji, pretesni čevlji, tujek v čevlju, neprevidno striženje nohtov – vraščanje, nepravilna pedikura in podobno),
  •  opekline ,
  •  pritiski debele poroženele kože (kalus) na tkivo pod njim,
  •  poškodbe s kemikalijami .

 

Večini teh ran in razjed, pri katerih je primarno zdravljenje neuspešno ali premalo uspešno, lahko pomagamo še z nekaj drugimi terapijami. Pomembna med njimi je HBOT – hiperbarična kisikova terapija.

Na podlagi mnogih medicinskih raziskav močno izboljša končne rezultate pri ranah, kjer je dotok krvi oslabljen ali celo prekinjen, kjer so prisotne otekline in/ali infekcija ali pri ranah, kjer imunski sistem iz različnih razlogov ne more opraviti svojega dela.

 

Fiziološke koristi HBOT – hiperbarične kisikoveterapije so:

 

 

  •  izboljšana oksigenacija ogroženih robov rane,
  •  generacija kolagena (pomembna beljakovina vezivnih tkiv, itd.),
  •  okrepljena fagocitoza ( celični proces, pri katerem tujke požrejo in razgradijo fagociti),
  •  izboljšano delovanje levkocitov,
  •  okrepljeno penetracijo določenih antibiotikov,
  •  izboljšano celjenje ran s pomočjo povečane proizvodnje fibroblastov
  •  kapilarna angiogeneza (fiziološki proces rasti novih krvnih žil),…
  •  neposreden bakteriostatičen učinek kisikove terapije na anaerobne mikroorganizme,…

 

 

Hiperbarična kisikova terapija pomeni dihanje 100-odstonega kisika pri kontrolirano povišanem tlaku. Posamezna terapija lahko traja tudi do dve uri. Odvisno od rane in samega stanja le-te je ponavadi potrebnih okoli 15-20 terapij, včasih celo 40. Ko je telo dovolj dolgo izpostavljeno hiperbaričnemu kisiku, se v telesu začnejo tvoriti nove kapilare (kapilarna angiogeneza), preko krvne plazme se olajša dovod kisika do poškodovanega področja, itn.
Zdravljenje diabetičnih in vseh ostalih ran, ki se slabo celijo, je zelo uspešno s pomočjo hiperbarične komore in čistega medicinskega kisika. Zelo pomembno je, da se hiperbarična terapija uporablja kot dodatno orodje ob standardni oskrbi ran in pod nadzorom usposobljenega zdravnika.

Pure medical oxygen or oxygen concentrator!?

Medical oxygen is the only kind of oxygen, used in any real hyperbaric medicine.

Hyperbaric oxygen therapy can be performed in HARD chambers and also in so called SOFT ones. And again, HARD chambers are the only kind of chambers, used in any real hyperbaric medicine. They can generate much higher preasure and are much, much safer than SOFT (inflatable) ones.

Term, hyperbaric oxygen therapy, can only be used, when a certain combination of elevated preasure and pure 100-percent medical oxygen is in play. Everything else is nothing but a bad substitute with absolutely NO place in hyperbaric medicine. Many researches have been done world wide, regarding oxygen therapy. Allof them with pure 100-percent medical oxygen. Not a single one with oxygen concentrator.

 

Basic mathematical facts explains us why.

Nowadays, best oxygen concentrators produce 5-6 liters of approx. 90-percent oxygen per minute. Let us round to the maximum.

6 liters of oxygen from concentraor * 60 minutes * 90% = 324 liters of xygen per hour.

Average air consumption of a person at rest is 600 liters of air per hour. Increased under load or activity, of course. Increased environmental pressure represents exactly that. Accordingly, air consumption is increased. In case of hyperbaric oxygen therapy, oxygen consumption is increased.

During basic therapy, every patient consumes 1000 liters of pure 100-percent medical oxygen per session (60 minutes). Equivalent with increasing pressure and duration of therapy, amount of oxygen is higher.

It is more than obvious that even the best oxygen concentrator  can not follow requirements of hyperbaric medicne. Adding approximately 700 liters of air to produced 320 liters of oxygen (to provide the required amount of 1000 liters per hour) shows us everything. Average percantage of oxygen during every hyperbaric therapy, using oxygen concentrators, NEVER EXCEEDS 30%. Patient can feel a little bit fresher because of that but we can not possibly talk about any real medical efficiency. Even more, long-term harmfulness due to the presence of impurities is very high.

 

 

Oxygen concentrator is a device that helps people breathe at normobaric atmosphere conditions. Patients with serious lung deseases or disabilities that can’t get enough oxygen into their bodies trough breathing air. In majority of such cases, lungs are incurable and their function is severely weakened. Patients with this kind of problems need oxygen generator so they can inhale air with much higher partial oxygen pressure. To summon things up, oxygen concentrator is a device that facilitates breathing and provides enough oxygen for people, who’s lungs are to weak to do that.

Laws of physics in hyperbaric medicine tells us more that just that. Pure oxygen (and most of other gases) dissolves at elevated pressures. It dissolves into blood plasma, spinal fluid,… and in this form much easier reaches all body tissues. Especially those with weakend or interrupted blood flow. Either due to injury or the nature of the tissue itself.

Producing oxygen with oxygen concentrator in combination with elevated air pressure has some more long-term negative consequences that we should take into consideration.

 

 

Air is a mixture of gases, mostly nitrogen and oxygen, is without color, taste and odor. It is naturally polluted by various hard particles like dust, sand, salt, lead, other heavy metals, etc.. This polution is in direct dependence on the specific area and on altitude. Naturally city air, heavily burdened with transport and industry, contains even more such impurities (allergens, lead,…).

Humidity is another factor to be considered. Whether high or low, it is allways present. If we pressurize such unfiltered air, we also linearly increase amount of impurities. At normobaric atmosphere, we all breathe them and they are not very healthy. The harmfulness of these is increasing exponentially with pressure. Therefore, it is extremely important that we use pure 100% medical oxygen in every hyperbaric treatment.

 

Medical oxygen is virtually free from impurities.

 

To achieve such purity, professional filtration systems are needed.

OXYGEN CONCENTRATORS DON’T HAVE SUCH FILTERS because they are not meant to be used under elevated pressure.

Hyperbaric oxygen therapy in orthopedics

Sports injuries, fracture repair, osteoporosis,…..one of the most common cosequences in more or less every orthopedic indication is a serious damage and reduction in microcirculation. Oxygen level in damaged area is predominantly very low. HBOT is believed to compliment the debridement process by improving oxygenation at the fracture site. This promotes revascularization and improves other host factors such as white blood cell oxidative killing, fibroblastic proliferation and osteoclastic activity. The osteoclast is known to be very oxygen dependent cell. HBOT appears to stimulate osteoclast function. The osteoclasts then become effective in clearing residual infected, non-viable bone whereas they were unable to do so in the hypoxic environment. Moreover, increasing the oxygen tension in hypoxic areas promotes synthesis of collagen, proliferation of fibroblasts and capillary angiogenesis.

Hyperbaric oxygen treatment involves intermittent inhalation of 100% oxygen at greater than atmospheric pressure. This treatment is administered in a hyperbaric chamber, which is compressed with air, while the patient breathes oxygen at ambient pressure through a mask. Inspiration of oxygen at high pressure increases the amount of oxygen dissolved in the plasma in direct proportion to the rise in ambient pressure. In such conditions, the plasma oxygen content increases from 0.32 volume percent to 4.8 to 5.76 volume percent. This high dissolved oxygen concentration improves oxygen delivery to bone tissue, reduces marrow edema, and thereby reduces intraosseous pressure, improving venous drainage and microcirculation. By flooding the extracellular fluid with diffused oxygen, the oxygen becomes available to the ischemic bone cells without the need for circulating hemoglobin.

Official studies confirmed highly positive results of adjuvant hyperbaric oxygen therapy, due to increased oxygenation and consequently edema reduction, minimized infections and serious reduction to post-injury tissue necrosis.

Effects of HBOT !

In short : if the basic underlying processes include low oxygen (hypoxia), loss of blood flow (ischemia), and/or secondary inflammation, HBOT can potentially help. HBOT’s versatility makes sense only when you look beneath the surface to find the common threads in all the conditions and diseases.

In Orthopedy, use HBOT as an adjuvant therapy as soon as possible, after the injury or surgery.

Ischemic stroke can heal. Scientifically proven!

Ischemic stroke represents approximately 85% of all brain strokes. Blood supply to part of the brain is decreased or even stopped. Consequently supply of affected brain area with oxygen and other nutrients (mostly glucose) is diminished,  leading to dysfunction of the brain tissue in that area. Ischemic stroke occures due to some sort of obstruction of a blood vessel, leading to neurological failure.

Hyperbaric oxygen chamber improves and accelerates recovery. The use of such oxygen therapy was scientifically confirmed. Substantial recovery of neurological functions in tissue, affected whether by brain stroke, traumatic injury or some other methabolic irregularities is  proven. Ischemic stroke, traumatic injury, and metabolic disorder are major causes of brain damage and permanent disabilities, including motor dysfunction, psychological disorders, memory loss, and more. The brain consumes 20 percent of the body’s oxygen, but that is only enough oxygen to operate five to ten percent of neurons at any one time. The regeneration process requires much more energy. The tenfold increase in oxygen levels during HBOT treatment supplies the necessary energy for rebuilding neuronal connections and stimulating inactive neurons to facilitate the healing process

 

Recent research even showed the effect of hyperbaric oxygen chamber as a result of direct influence on DNA. Due to oxygen therapy, stimulation of certain genes occures. Genes that affect growth, recovery mechanisms and anti-inflammatory activity in human body.
